The impact of cognitive and affective disorders on clinical characteristics and quality of life in elderly chronic heart failure patients

The goal of the work was to study clinical functional features and life quality in elderly patients with chronic heart failure in accordance with cognitive disorders and abnormalities in emotion field. 106 patients with chronic heart failure II–III NYHA were examined. MMSE test, Beck test, Hamilton and Spielberger-Khanin scores, SF-36 questionnaire to analyze the quality of life were included in the study.
Moderate cognitive deficiency was observed in 34 patients, the vascular dementia was seen in 37 patients, anxiety disorders were revealed in 35 patients. Prognostically unfavorable ventricular arrhithmias was reliably diagnosed more often in patients with anxiety disorders. These patients did reliably less distance on the 6-minute-walk test and it was 115,9±17,7 (95 % CI 109,8–121,9). The results of the study revealed that patients showed not only the physical component but, moreover, major problems in their psychological, emotional and social functions.
Patients with dementia showed the most expressed reduction in indices according to scales of physical role functioning, emotional role functioning and mental health. The lowest quality of life parameters according to the SF-36 were 8.3±2,6 (95% ДИ 6.9–10.5) were revealed in patients with anxiety and depressive disorders. Social functioning disorder in most cases depends on patients' failure in socializing.
Quality of life parameters may be further used in complex research of old-age patients with chronic heart failure in order to increase the efficiency of individual healthcare.
