Quality of life in patients after reconstructive surgery for benign strictures of bile ducts

Reconstructive surgery is the main approach of treatment of benign bile strictures. Now one of perspective approaches for complex evaluation of health status in patients with benign bile strictures both in immediate and remote periods after reconstructive surgery is quality of life (QoL) assessment. The goal of this controlled clinical trial was to determine the efficacy of new surgical techniques in patients with benign strictures of bile ducts in terms of QoL. For QoL assessment the general standardized SF-36 questionnaire and the special GSRS questionnaire were used. As the result, all of the QoL parameters (before surgery, in 3, 6 and 12 months after reconstructive surgery on bile ducts) in the group with the innovative approach were superior to the group of patients with classical approach. In 12 months after surgery parameters of abdominal pain, reflux syndrome and constipation syndrome decreased in patients who received innovative approach and become similar to those patients without benign strictures of bile ducts. Implementation of instruments for QoL assessment in clinical practice gives the opportunity to determine the advantages of innovative approach in the management and treatment of surgical patients. Also, the analysis of complications in patients after stricture of bile ducts has been done. Quality of life in these patients had a statistical relationship with type of performed reconstructive surgery, course of the recovery period and prognosis of the health state. QoL assessment provides the continuity of patient management, improves the effectiveness of outpatient monitoring, eliminates interethnic differences during interactions with patients, allows to investigate the influence of various chronic diseases on a person from new perspectives, simplifies the examination of new approaches for treatment of chronic diseases. In conclusion, the general SF-36 questionnaire and the special GSRS questionnaire should be included in the national protocol for conducting reconstructive surgery for benign stricture in the bile ducts at the level of the Ministry of Health in the Republic of Moldova.
