Quality of life in patients with arterial hypertension combined with chronic cerebrovascular diseases when using electrophoresis with Mexidol

FEDERAL RESEARCH CENTER FOR FUNDAMENTAL AND TRANSLATIONAL MEDICINE, NOVOSIBIRSK

Arterial hypertension (AH) is often complicated by comorbid pathology ― cerebrovascular diseases (CVD). Significant functional disorders (cognitive, motor, psycho-emotional, etc.), reduced quality of life and worse prognosis in AH complicated by CVD necessitate rehabilitation measures in this category of patients. Purpose of the study is evaluat ion the effectiveness of application of electrophoresis with Mexidol in the rehabilitation of patients with arterial hypertension combined with chronic CVD (CCVD) by the dynamics of clinical parameters and quality of life indicators. A randomized placebo-controlled study was conducted at the clinic of the Federal Research Center for Fundamental and Translational Medicine (Novosibirsk). 53 patients with hypertension combined with CCVD, men and women, aged 20 to 75 years, were examined and treated. Patients were divided into 2 groups matched by age, gender, main clinical manifestations of the disease: main (n=29) and control (n=24). Patients of the main group, under the background of basic drug therapy, underwent a course of rehabilitation by electrophoresis with Mexidol. Patients in the control group received only basic drug therapy. Evaluation of the effectiveness of the prescribed rehabilitation method was carried out according to the dynamics of the clinical condition, blood pressure, quality of life indicators and psycho-emotional indicators. The validated SF-36 questionnaire was used to assess health-related quality of life. The hospital anxiety and depression scale (HADS) was used to assess the psycho-emotional state (anxiety/depression). Electrophoresis with mexidol was carried out from the apparatus «Elfor» according to the collar technique. Course ― 10-15 procedures. A comparative analysis revealed statistically significant differences in the dynamics of clinical manifestations, hemodynamic parameters, indicators of anxiety and depression, and quality of life after a course of treatment and rehabilitation measures in the main and control groups. The use of electrophoresis with Mexidol led to a more pronounced decrease in the clinical manifestations of hypertension and CCVD, a decrease in blood pressure values, a decrease in anxiety and depression, and an improvement in the quality of life. Using of this rehabilitation method led to a significant increase in indicators on the scales of physical functioning, role emotional functioning, mental health, physical and mental health components. The efficiency of using electrophoresis with Mexidol in patients with hypertension combined with CCVD has been established, which makes it possible to recommend this physiotherapeutic method for the rehabilitation of this group of patients at the hospital stage.
